Professional Partners

Carers’ Support work very closely with other voluntary sector organisations and charities, Social Services and Health services, promoting carers awareness and providing information, advice and guidance at Information events, annual general meetings and community events.

We are always happy to provide information desks, speakers and presentations about carers and the work that we do to help to support them. Contact Keeley Taylor on 01233 664393 if you would like us to join you.

Professionals in Health and Primary Care can refer carers into our

Rapid Access for Carers Support Service :

Approximately 12% of the patients who are registered at your GP or Medical Practice could be looking after someone at home, who depends on them.

Criteria

Rapid Access to Carers Support can be actioned, where the carer’s health & wellbeing could be improved by putting into place a service, that will enable them to continue caring at home (preventing carer breakdown and hospital admissions or readmissions).

How we help

Responding within 24 hours, we act as brokers and put into place the most appropriate service.

Services that we can access

Replacement care (i.e. Crossroads Care), nursing care at home, day care centre place, domiciliary care (anything from making sure someone takes their meds, getting someone up and dressed in the morning to 24 hour care), residential /nursing home. Domestic help -general house work duties including ironing and cleaning services, someone to do the shopping.

Most services will be met with up to £400 but, If the only way forward is through a place in residential or live-in care, we can spend up to £800 specifically on this. (Funded by the local Clinical Commissioning Groups)

Research shows that carers who are well informed and supported are less demanding on their GP and Primary Care Team.

Carers who provide high levels of care are more than twice as likely to suffer from poor health. Enabling them to access specialist support can prevent them from becoming high intensity users of health services.

Our staff will liaise with other services, agencies and professionals for carers relieving health professionals of the need to do this.

Good information about the condition of the person they care for is one of the most important things for carers. We can help by providing general information about health conditions and referring carers on to other organisations (eg. Alzheimer’s Society, Stroke Association). Accessing our services can relieve carers of some of the stress they carry due to their caring role, encouraging better health and well being.

Help for Carers of Mental Health Patients

If you have patients who are looking after someone with a Mental Health issue we can give 1-2-1 support. Elena Ward, previously a worker for Rethink, will be happy to make an appointment with the carer. Please refer the carer to us in the usual way.

The impact of a carers role on their health will become less and therefore they will spend less time visiting their GP, as their health and well being improves through our support.

With the carer in a better state of mind and health they will be able to continue caring at home for longer, be better equipped both practically and emotionally for their caring role and in turn prevent admissions or re-admissions to hospital and bring GPs stats down around Long Term Conditions and mental health.
